What is Transcranial Magnetic Stimulation (TMS)?

TMS is a relatively new therapy in which a qualified clinical uses a metal coil designed to stimulate the brain’s neurochemicals by delivering magnetic pulses to the scalp. During this noninvasive procedure, several pulses are administered to the brain in succession via electric currents transmitted through the coil. 

This approach is significantly different from the use of antidepressant medication because it directly (versus indirectly) activates the brain’s messaging center believed to be dysfunctional among those with behavioral health disorders such as depression.

Mental Health Disorders That May Be Treated With TMS Include:

Currently, the Food and Drug Administration allows TMS to be marketed as a treatment for major depression, obsessive-compulsive disorder (OCD), and certain types of migraine headaches.1 However, its off label uses have included:

  • Anxiety disorders
  • Bipolar disorder (BD)
  • Post-traumatic stress disorder (PTSD)
  • Neurological disorders
  • Substance use disorders
  • Schizophrenia and schizoaffective disorders
  • Other mood, anxiety, and trauma-related disorders 

What is Treatment-Resistant Depression (TRD)?

TMS is often considered a first-line approach for treatment-resistant depression. TRD is a condition in which a person’s depressive symptoms are not adequately treated by antidepressants or other common approaches. Moreover, despite having received appropriate treatment, symptoms persist or only partially improve.

Those diagnosed with TRD have typically undergone at least two trials of antidepressant treatment (e.g., selective serotonin reuptake inhibitors aka SSRIs) without experiencing significant relief from their symptoms. 

Symptoms of Depression or TRD Include:

  • Persistent feelings of sadness or low mood
  • Irritability or agitation
  • Loss of interest or pleasure in activities
  • Fatigue and low energy
  • Adverse changes in sleep patterns
  • Changes in appetite leading to weight loss or gain
  • Suicidal thoughts or behaviors

TRD can also result in significant impairments in daily functioning, including relationship strain, social withdrawal, or occupational difficulties. Those with TRD may also be at a heightened risk of relapse, as it can be particularly challenging to manage this condition.

Is TMS Treatment Safe?

Generally speaking, TMS treatment is considered safe and well-tolerated by patients. When administered by a qualified behavioral health professional, such as a psychiatrist, few known side effects during or after TMS that warrant concern. Rarely, seizures can occur, and the risk is low. The safety of TMS during pregnancy or breastfeeding is not entirely known, but research is ongoing.

Can I Drive Home After Treatment?

Following treatment, you are free to drive yourself home and resume your regular activities, as side effects are brief, mild, and not expected to cause any significant impairments.

How Long is TMS Treatment?

The length of TMS treatment plans can vary depending on your specific condition and response to treatment. But they typically last between 20 to 30 sessions. 

How Long are TMS Sessions?

In general, TMS sessions last from 20 to 40 minutes, but some newer TMS treatments use a technique known as theta burst stimulation, which may shorten the session to no more than 10 minutes.
