How to Request Medical Records from Guardian Recovery

If you need to request medical records from Guardian Recovery Network Holdings LLC or any of its related subsidiaries, follow these steps:
Step 1: Send an Email Request
Email your request to: [email protected]
Step 2: Complete the Authorization Form
You will receive a PDF form titled “Authorization to Release Protected Health Information.”
Fill out the form completely and accurately
Step 3: Return the Completed Form
Send the completed authorization form back via email to the same address
Step 4: Access Your Records
Once your request is processed, you will be provided with access to a secure portal where you can view and download your medical records
Important Notes:
Third-Party Requests: If someone other than the client is requesting records, they must provide either:
A signed authorization from the client, OR
Appropriate legal documentation (such as a court order or power of attorney).
This process applies to all Guardian Recovery facilities and subsidiaries nationwide
