Florida

Substance use disorder and mental health facilities in Florida are regulated by the Department of Children and Families (DCF) through the Substance Abuse Program Office. You can contact them at 1-866-762-2237 or visit their website at https://www.myflfamilies.com/service-programs/samh/. New Jersey

The New Jersey Division of Mental Health and Addiction Services (DMHAS) oversees substance use and mental health treatment facilities. Contact them at (609) 777-0702 or visit https://www.nj.gov/humanservices/dmhas/. You can also reach the main Department of Human Services at 1-877-652-7624 for general inquiries about licensed facilities. Texas

The Texas Health and Human Services Commission (HHSC) licenses substance use disorder treatment facilities and mental health services through Health Care Facilities Regulation (HCFR). New Hampshire

The New Hampshire Department of Health and Human Services, Bureau of Drug and Alcohol Services licenses substance use disorder treatment programs. How to File a Consumer Complaint

Florida

To file a complaint about a substance abuse or mental health facility in Florida, contact the DCF Abuse Hotline at 1-800-96-ABUSE (1-800-962-2873). You can also file a complaint online through the DCF complaint portal at https://www.myflfamilies.com/service-programs/abuse-hotline/ or mail written complaints to: Florida DCF, 1317 Winewood Blvd., Tallahassee, FL 32399. New Jersey

File complaints with the DMHAS Complaint and Incident Reporting System by calling (609) 777-0702 or emailing [email protected]. You can also submit written complaints to: NJ DMHAS, 5 Quakerbridge Plaza, PO Box 362, Trenton, NJ 08625. The Division investigates all complaints regarding licensed facilities and takes appropriate action based on findings. Texas

To file a complaint about a licensed health care facility in Texas, call the HHSC complaint hotline at 1-800-458-9858. You can also file a consumer complaint with the Office of Attorney General’s Consumer Protection webpage at https://www.texasattorneygeneral.gov/consumer-protection. Maine

File complaints about substance use disorder or mental health facilities by contacting the Maine DHHS Division of Licensing and Certification at (207) 287-9300 or emailing [email protected]. Written complaints can be mailed to: DHHS Division of Licensing and Certification, 41 Anthony Avenue, 11 State House Station, Augusta, ME 04333. New Hampshire

Submit complaints to the NH DHHS Bureau of Drug and Alcohol Services by calling (603) 271-6110 or emailing [email protected]. Complaints can also be filed with the Office of the Long-Term Care Ombudsman at (603) 271-4375 if applicable. Mail written complaints to: NH DHHS, 105 Pleasant Street, Concord, NH 03301. Colorado

To file a complaint about a licensed substance abuse or mental health facility, contact the Wisconsin DHS Bureau of Prevention Treatment and Recovery at (608) 266-2717.
